Brief 0 - Final questionnaire

Here is the final design and site for my branding questionnaire. My friend managed to incorporate some extra interactivity that enhanced the UI — the multiple choice answers fade out when 3 answers are selected and the "Thankyou!" text only appears when the user clicks submit. These are little additions but hopefully it will enhance the site.
